Travel Plan

Please fill out a Travel Plan when planning youth activities that take place outside of Salt Lake County at least two weeks before the activity. Best practice is to submit the Travel Plan as soon as the activity is planned.

Review section 20.7 of the Handbook to ensure your activity is in harmony with the safety policies and guidelines. 

Permission and Release Forms (link in main navigation) are required from all youth participants for any activity leaving stake boundaries.

When an activity requires traveling over 100 miles or is overnight, please fill out the Travel Plan AND an Event and Activity Plan.
